New Patient Forms
Your First Dental Visit
Just like every other appointment you’ll experience here, your first visit will be all about YOU. After being greeted in our reception area (we don’t have a “waiting room” because our patients don’t wait!), you’ll be given a brief tour of the dental office so you can meet the team, and then we’ll begin the appointment by taking a CBCT scan (3D X-ray) of your mouth and head. Dr. Timmerman will ask you a simple question: “Why are you here today?” Do you just need a checkup and cleaning? Some restorative dentistry? Looking to improve your appearance? Based on your answer, Dr. Timmerman will come up with a customized treatment plan designed to turn your goals into a reality.

While dental insurance can seem complicated at times, here, we like to keep it nice and simple. If you have a plan, we’ll accept it, and not only that, our team will also file your claim for you to make sure you’re maximizing your benefits.
